Information for Visiting Physicians

Dear Prospective Visiting Physician:

Thank you for your interest in taking a rotation with us at Rush University Medical Center. On a space available basis, each residency or fellowship program may allow visitors to participate in a rotation under their supervision. Please contact the programs directly for the feasibility of your timing and request.

All visiting physicians must be currently in an ACGME-approved training program in their home institution AND:

  1. be licensed to practice medicine in the State of Illinois

    If you hold a permanent (#036) license or a temporary training (#125) in Illinois and are currently in an ACGME-approved residency/fellowship training program, we will need a copy of it.

    If you are not licensed or licensed from another state, you will need to apply for an Illinois license to practice medicine, even if only for one month. There are two options:

    • A temporary limited medical license (PDF, 323K), which costs $100 and only covers the exact dates of the rotation here.
    • Please note: There are additional forms that need to be completed for International Medical Graduates currently in ACGME-approved training programs. Contact GME for more information (312 942-9551)

    • A permanent license which costs $300 and is good for up to three years (dependent upon the expiration date; all issued prior to June 2011 will expire on July 31, 2011 when the new 3-year cycle begins again). The permanent license is only a good option if you plan on moving/staying in Illinois or returning often. Please read the description to be sure that you qualify first. If you are unsure as which to apply for, please contact us.

    Both sets of forms may be opened from the links and completed directly from your computer. Then print them out for submission to Rush GME. Be sure to sign and date page 4 of the application as well as any other forms requiring a signature.

  2. complete the application form for a visiting physician (PDF, 79K)

    You will need to complete the applicant information on the form (with the exception of the two boxes labeled for internal use only). Please print it from your computer and print or type clearly.

    Note that the completed form with ALL required documents MUST be received in GME at least 3 weeks PRIOR to the start date of the rotation FOR THOSE ALREADY HOLDING A VALID ILLINOIS LICENSE.

    For visiting physicians who are not currently licensed in Illinois, it will take about 3 MONTHS notice to apply and obtain a license.

  3. supply all the necessary documentation (a checklist is provided on the application form for your convenience):
    • a copy of your Illinois medical license (if you currently have one)
    • a copy of a current CV or previous ERAS® application
    • a letter of permission from your home institution/program director
    • verification of your malpractice insurance coverage from your home institution
    • a copy of a valid ECFMG certificate (if applicable)
    • proof of your completion of a HIPAA program at your home institution
    • copy of your immunization records or health clearance form from your Employee Health Office

Please return the completed form and attachments to the contact person you have been working with from the Rush program you wish to visit. Once the department approves the rotation schedule (and forwards the completed form and all required attachments to GME), you will receive final notification by email from your contact person.

If your program requires a formal agreement, you may have that sent to the contact information below for the signature of our Dean.

Following that, we suggest you make make arrangements for your orientation and first day meeting time and location with your program contact person.

When you arrive, the department program coordinator will have a copy of your approved application form, which will have your Rush physician ID number on it. The completed form will also be your approval for Human Resources to make your temporary Rush ID badge. This form should also be brought to the Rush parking garage office to receive discounted student parking rates (currently $40/month). A $25 refundable deposit will be added to this cost (payable by cash or check) on your first day at Rush. Enter the visitor's parking entrances (either A or B), and take your receipt with you to the parking office as well. Once you have your AVIT (transponder), you must park in the Employees entrances (C through F). At the end of your rotation, you must return the AVIT and you will receive your deposit back.
